'Chameleon' Testing and Diagnostic Kit Order Form

The order form below should be filled out electronically (without submitting), printed out, and mailed to the address listed on the left hand side of the page.

Each 'Chameleon' Kit contains four Flat Clips, four Round Clips, two standard Spiral Wraps, two micro Spiral Wraps, one Releasable Cable Tie, one 'Danger' Label, three Hook-Up Wires and a detailed instruction sheet.  Each pack cost 25 U.S. dollars.

Shipping and Handling is 5 U.S dollars for each kit ordered. Please allow 7-10 days after receipt of order for delivery. Pennsylvania addresses must add 7% sales tax on to the total, including the shipping and handling.

We only accept money orders and checks; credit cards will be available at a later date.  Money orders and checks can be made out to HazardGuard Safety Wire, Inc.
